Doctors Warn of AI Dependency in Colonoscopy: Boost or Blind Spot?


Artificial intelligence has rapidly become an essential partner in modern medicine, promising to enhance precision, reduce human error, and save lives. In fields like gastroenterology, AI-powered tools now assist doctors in detecting adenomas—precancerous polyps that can develop into colorectal cancer. Yet a new study highlighted by NPR raises a sobering question: what happens when doctors grow too dependent on AI? The findings suggest that while AI boosts detection rates during use, doctors’ independent skills may deteriorate when the technology is absent.


The Study Behind the Concern

The concern originates from research published in The Lancet Gastroenterology & Hepatology. Over several months, endoscopy centers in Poland tracked the performance of experienced physicians before, during, and after they began using AI systems for colonoscopies.

  • Before AI: Physicians achieved an adenoma detection rate (ADR) of 28%.
  • With AI assistance: ADR rose to 25.3–28%, depending on the study sample.
  • After AI was removed: ADR fell to 22%, well below pre-AI levels.

This decline suggests not just the absence of AI’s supportive role, but a possible erosion of the physicians’ natural diagnostic sharpness—a phenomenon researchers describe as a form of “deskilling.”


The “Google Maps Effect” in Medicine

Experts liken this pattern to the “Google Maps effect.” Just as frequent GPS users can struggle with navigation without digital guidance, physicians who rely too heavily on AI may lose their ability to recognize subtle visual cues on their own.

Colonoscopy requires a high degree of vigilance and pattern recognition. Even small adenomas can pose future cancer risks, and a missed detection can have serious consequences. Every 1% increase in ADR is associated with roughly a 3% reduction in colorectal cancer risk. That means even modest declines in detection can have population-level implications for cancer prevention.


Balancing the Benefits and Risks of AI

While the findings are concerning, they do not discredit AI outright. In fact, AI has demonstrated clear benefits in several areas of colonoscopy:

  • Enhanced support for junior doctors: AI can help less experienced physicians identify harmless polyps that might otherwise cause uncertainty, speeding up training and reducing false positives.
  • Reduced fatigue: Colonoscopies are repetitive and mentally taxing; AI offers a reliable second set of “eyes,” particularly valuable during long procedures.
  • Standardized care: AI may help reduce disparities between doctors with varying levels of experience or training.

Still, the potential for skill erosion demands caution. If AI systems fail, doctors must be able to revert seamlessly to independent judgment. Overreliance could compromise patient safety in precisely those moments when human expertise matters most.


What Professional Guidelines Say

The American Gastroenterological Association (AGA) has not issued definitive guidance recommending for or against AI use in colonoscopy. Instead, it acknowledges both the promise and the uncertainties. Current guidelines emphasize:

  1. Careful integration: AI should supplement, not replace, human judgment.
  2. Ongoing training: Doctors must continue performing procedures without AI to maintain their observational skills.
  3. Rigorous evaluation: Long-term studies are needed to measure whether AI improves cancer prevention outcomes, not just detection metrics.

Meta-reviews of AI in colonoscopy further highlight these gaps, calling for stronger data on real-world impacts and risks of deskilling.


A Cautious Path Forward

The NPR report underscores a paradox: the very tool designed to improve patient safety might—if misused—undermine the skills that keep patients safe. The path forward lies in balance. Doctors and hospitals must:

  • Use AI as a supportive assistant, not a crutch.
  • Build training regimens that require doctors to alternate between AI-assisted and unassisted procedures.
  • Encourage ongoing research into the long-term consequences of AI integration in medical practice.
