From childhood, she saw human faces transforming into dragon-like creatures—complete with elongated snouts, reptilian skin textures, large pointed ears, and vivid, brightly colored eyes. These weren’t fleeting illusions but persistent, haunting distortions that appeared everywhere: in the faces of loved ones, in people she passed on the street, and even on inanimate objects like walls or electrical sockets. In the dark, when faces weren’t visible, the dragon-like images seemed to rise up from nowhere, surrounding her world.
This unsettling experience baffled doctors for years until a neurological explanation finally emerged: a condition called prosopometamorphopsia (PMO).
What is Prosopometamorphopsia (PMO)?
Prosopometamorphopsia is an extraordinarily rare neurological disorder where people perceive faces as grotesquely distorted. Instead of recognizing a familiar human face, the brain misprocesses the visual information, warping it into something monstrous or unnatural. For some patients, this distortion manifests as drooping features, misaligned facial structures, or exaggerated proportions. In this woman’s case, the faces became dragons.
Because of its eerie and frightening presentations, PMO has sometimes been nicknamed “demon face syndrome.” But in medical terms, it reflects a malfunction in the brain’s facial recognition pathways, likely involving the ventral occipitotemporal cortex—a region responsible for processing both face shapes and colors.
To date, medical literature records only around 75 cases worldwide. Some people experience the distortions across the whole face, while others have hemi-PMO, where only one side of the face warps. Such rarity makes each case a valuable opportunity for doctors to better understand how the human brain interprets faces.
The Neurological Puzzle
When specialists examined the woman, they ran brain scans to uncover possible abnormalities. An MRI revealed small lesions in brain areas that regulate facial recognition and color processing. Although her EEG (which tracks electrical activity) appeared normal, neurologists suspected the hallucinations stemmed from abnormal neural firing in the ventral occipitotemporal cortex.
This theory aligns with past research linking PMO to structural or electrical disruptions caused by epilepsy, migraines, strokes, or brain injuries. In this woman’s case, the lesions acted as faulty wiring, scrambling the signals her brain used to interpret faces. Instead of recognizing her friends, family, or even strangers, she encountered dragons.
The Treatment Journey
Because PMO is so rare, doctors don’t have a standard treatment plan. Instead, they experiment with medications that might stabilize the brain’s visual processing.
- Valproic Acid – an anticonvulsant commonly prescribed for seizures and migraines – was the first treatment tried. Remarkably, it worked: the dragon faces vanished. For the first time in her life, she saw ordinary human faces without distortion.
- But there was a downside. Soon after, she began experiencing auditory hallucinations, hearing loud banging noises while trying to sleep. To address this, doctors switched her medication to rivastigmine, a drug usually prescribed for dementia. Surprisingly, this not only reduced the auditory disturbances but also helped keep her visual hallucinations under control.
Her case highlighted both the complexity of PMO and the delicate balance of brain chemistry that treatments must achieve. Some patients with PMO have tried antidepressants or antipsychotics with little success, making this treatment path especially significant.
A Broader Context of Visual Distortions
While PMO remains exceptionally rare, it shares features with other unusual neurological conditions. For example:
- Alice in Wonderland Syndrome (AIWS): People experience warped perceptions of size and shape, making objects appear tiny, giant, or oddly stretched.
- Charles Bonnet Syndrome: Patients with vision loss sometimes experience vivid hallucinations of faces, animals, or patterns.
- Prosopagnosia (Face Blindness): Unlike PMO, this condition prevents people from recognizing faces at all, though they still appear normal.
These conditions remind us that our visual world isn’t purely external—it’s also a construction of the brain. When the brain’s circuitry misfires, reality itself can appear transformed.
Why This Case Matters
The dragon-vision case underscores how much we still have to learn about the human brain. With so few known cases of PMO, every diagnosis provides new clues about the neural pathways that allow us to recognize one another. Beyond scientific curiosity, such insights can improve treatment options for people who live with terrifying perceptual distortions.
For the woman at the center of this study, finally receiving a diagnosis was life-changing. Not only did it validate her unusual experiences, but effective treatment also allowed her to see people as they really are—for the very first time.
