Type 5 Diabetes: The Long-Overdue Recognition of a Distinct Disease


For decades, diabetes has been broadly divided into a handful of categories—Type 1, Type 2, gestational, and a few rare genetic subtypes. These classifications shaped medical research, treatment plans, and public health campaigns. But today, the global medical community officially acknowledges a new category: Type 5 diabetes. This recognition by the International Diabetes Federation (IDF) marks a historic step in understanding the diversity of diabetes and addressing a form that has been overlooked for too long.


What Exactly Is Type 5 Diabetes?

Type 5 diabetes, also referred to as malnutrition-related diabetes mellitus (MRDM) or severe insulin-deficient diabetes (SIDD), arises from a very different root cause than the familiar types. Unlike Type 1, which is triggered by an autoimmune attack on insulin-producing cells, or Type 2, which develops through obesity-driven insulin resistance, Type 5 stems from chronic undernutrition during early life.

This undernutrition impairs the proper development of the pancreas, leaving individuals with a limited capacity to produce insulin. The condition often strikes lean adolescents and young adults, particularly in low- and middle-income regions across South Asia, sub-Saharan Africa, and parts of Latin America.

While medical literature has long described cases of such diabetes—often in young, underweight individuals—the absence of a clear classification meant millions were either misdiagnosed as Type 1 or Type 2, leading to inappropriate treatment plans and devastating complications.


A History of Medical Neglect

The roots of Type 5 diabetes in medical research stretch back nearly 70 years. In the 1950s and 1960s, clinicians in India and Africa began reporting unusual diabetes cases among malnourished youth. These patients didn’t fit the patterns of either Type 1 or Type 2, but the science at the time lacked the clarity to define a new category.

In 1985, the World Health Organization (WHO) briefly recognized malnutrition-related diabetes mellitus (MRDM) as a subtype. Yet, by 1999, this designation was removed due to “insufficient evidence.” The result was two decades of silence—while the patients themselves continued to exist, largely invisible to policy makers and research funding agencies.

The turning point came in January 2025, when an international panel of diabetes experts met in India. After reviewing decades of clinical and epidemiological data, the panel unanimously agreed to classify malnutrition-related diabetes as a distinct entity. The proposal was endorsed in April 2025 during the IDF’s World Diabetes Congress in Bangkok, and the decision has now been publicly announced worldwide.


How Is Type 5 Different From Other Diabetes Types?

Though sharing surface similarities with Types 1 and 2, Type 5 diabetes stands apart:

  • Not autoimmune: There is no immune attack on the pancreas, unlike Type 1.
  • Not obesity-related: Insulin resistance from excess body weight is absent, unlike Type 2.
  • Triggered by poverty and malnutrition: The disease reflects developmental insults from undernutrition, often starting in childhood.
  • Distinct clinical profile: Patients are usually young, lean, and from resource-limited communities.
  • Different treatment response: Many respond better to low-dose insulin or oral drugs, while full insulin regimens can trigger dangerous hypoglycemia.

The IDF estimates that 20–25 million people worldwide may be living with Type 5 diabetes—making it a major, yet largely unrecognized, global health issue.


Why Recognition Matters

1. Better Diagnosis

Until now, many Type 5 patients were misclassified as having Type 1 diabetes and placed on intensive insulin therapy. But unlike true Type 1 patients, their insulin requirements are lower, and heavy doses can actually cause life-threatening drops in blood sugar. Recognition allows doctors to apply more nuanced diagnostic criteria and prevent harmful mismanagement.

2. Tailored Treatment

With Type 5 now defined, researchers can develop treatment guidelines specific to this group. Oral medications, dietary support, and carefully calibrated insulin may become the standard approach.

3. Equity in Global Health

Type 5 overwhelmingly affects marginalized populations in low-resource settings—the very communities most often left out of medical research. By acknowledging it, global health bodies signal that the needs of these populations matter.

4. Research Momentum

Recognition unlocks funding, clinical trials, and epidemiological studies. Instead of being lumped into other categories, Type 5 can be studied on its own terms, leading to better outcomes for millions.


The Road Ahead

The IDF has created a Type 5 Diabetes Working Group, led by Dr. Meredith Hawkins of the Albert Einstein College of Medicine in New York and Dr. Nihal Thomas of Christian Medical College, Vellore, India. Their mandate includes:

  • Establishing formal diagnostic criteria so doctors can accurately identify patients.
  • Developing global treatment guidelines suitable for resource-limited settings.
  • Building a registry of patients for long-term study.
  • Training healthcare workers to recognize and manage the condition.

The working group aims to release comprehensive guidelines within two years, potentially revolutionizing diabetes care for millions of underserved individuals.


A Paradigm Shift in Understanding Diabetes

The formal recognition of Type 5 diabetes underscores a profound truth: diabetes is not a single disease but a spectrum of disorders with diverse causes. For decades, global health has focused disproportionately on Type 1 and Type 2, both more visible in wealthier countries. But in doing so, it overlooked millions of people whose diabetes looked different—people whose suffering was often compounded by poverty, malnutrition, and lack of access to care.

By acknowledging Type 5, medicine is not only catching up with science, but also making a moral statement: no patient population should remain invisible.


The story of Type 5 diabetes is a reminder that diseases are shaped not just by biology but by social and economic realities. Malnutrition, poverty, and health inequality leave lasting imprints on the human body. Recognizing this condition is an act of both science and justice—one that may finally bring proper care, dignity, and hope to millions who have long lived in the shadows of misdiagnosis.

In the coming years, as diagnostic criteria and treatment guidelines evolve, Type 5 diabetes could become a pivotal case study in how global health learns to see the unseen.
